Our clinic is open Monday through Friday, from 8 am to 5 pm. Please call for an appointment. If you anticipate health maintenance laboratory work, do not eat or drink anything for 12 hours prior to your scheduled appointment. We encourage you to be a full partner in your health care experience.

Please schedule any anticipated Health Maintenance (such as lab) to be completed prior to your appointment. Point out ahead of your appointment if administrative form completion is required.

As a Dickson Street Clinic Patient, You Are Responsible for:

Providing, to the best of your knowledge, accurate and complete information about present
     complaints, past illnesses, hospitalizations, medications, alternative therapy and other
     matters relating to your health

Reporting unexpected changes in your condition to your doctor or nurse

Informing your doctor or nurse if you do not understand the plan of treatment and what is
     expected of you

Knowing and following the treatment plan prescribed by your medical team

Making and keeping appointments and notifying the appropriate staff when unable to do so

Accepting responsibility for outcomes related to refusing treatment or not following the
     medical team's instructions

Paying your bill/co-pay at the time of service

Keeping your contact information current and returning for and scheduling follow up visits to discuss laboratory, radiography or consultation results

All patients on Opiate Benzodiazepines, ADD/ADHD medications are required to have periodic unscheduled urinary drug screenings on a regular basis for documentation of compliance, non-divergence, and presence of non-prescribed medicines.


Our facility participates in the Arkansas Prescription Monitoring Program (APMP).
