It is with sincere regret that Dickson Street Clinic will be closing, 10/09/2020, due to losing our lease at 102 West Dickson Street.
It is very important that you continue to receive medical care. Part of the smooth transition of care includes the disposition of your treatment records. To assist you in that effort, enclosed is a release of records that must be signed by you and returned to my office before your request can be completed. I will need your new doctor’s name and address. After I receive this information I will forward your records to your new doctor without charge. However, if you request copies of your medical record this can be granted for a charge of: $0.50 for the first 25 pages and $0.25 for each additional page along with a $15.00 labor charge. We ask that you allow time for copying and for record transfer of your treatment records. We will not be able to grant same day request.

Both our current APRN and MD will be relocating in the near future.
Once, again, I am sorry for any inconvenience this may cause you, and I will work with you to make this transition as smooth as possible. I appreciate the time you’ve spent as my patient.
